The AI Cure-All: A Tall Order for Global Health

The Gates Foundation’s $1 billion investment in AI to improve healthcare, education, and agriculture in developing countries is a promising prospect, but also a gamble that raises more questions than answers. Bill Gates’ optimism about the potential of artificial intelligence to augment quality care is infectious, yet we must be cautious not to conflate innovation with infrastructure.

The partnership between the Gates Foundation and OpenAI was followed by a $200 million deal with Anthropic, and now a total of $1 billion will be allocated over two years to scale up AI interventions across various sectors. This massive investment assumes that AI can compensate for the lack of physical infrastructure in poorer countries, providing a digital safety net.

However, most AI interventions require some layer of physical infrastructure – roads, schools, power, and broadband – to function effectively. Satellite imagery has shown that rural communities in sub-Saharan Africa went dark after USAID cuts, making online education and weather apps inaccessible.

The Trump administration’s $150 million partnership with Zipline is a case in point. The company’s drones are saving lives by transporting medical supplies, but they still rely on staffed clinics to visit. As foreign aid cuts continue to decimate healthcare systems, the underlying infrastructure that these AI interventions depend on is crumbling.

One tool developed by the Gates Foundation improved medical diagnoses by 16 percent across several Kenyan clinics. However, this success story raises questions about whether such interventions will stop short of addressing the underlying issues. Enoh Ebong, president of the global development department at the Center for Strategic and International Studies, warned that these innovations may not be enough.

The relationship between AI and infrastructure is complex. While drones can deliver medical supplies, they still need roads to fly on. Similarly, digital tools require a basic level of literacy and electricity to function. Mark Suzman, CEO of the Gates Foundation, acknowledged “very real risks and challenges” surrounding AI, but believes it presents genuine opportunities for tangible improvements.

The question is whether we’re investing in innovation that complements existing infrastructure rather than replacing it. This requires traditional funding for education and healthcare to continue alongside AI interventions. In Rwanda, where Zipline’s drones are already saving lives, there’s a pressing need to rebuild the very infrastructure these high-tech deliveries rely on.

Without staffed clinics to visit, the full potential of these interventions will be wasted. In countries like America, where AI has yet to make a meaningful impact on people’s daily lives, we mustn’t get too caught up in the hype. The Gates Foundation’s bet on AI is necessary, but also a reminder that innovation alone cannot solve the world’s most pressing problems.

We need to be honest about what AI can and can’t do – and invest in both the technology and the underlying infrastructure that makes it work.

    While AI can augment care in resource-constrained settings, we're forgetting that digital solutions often require analog precursors – physical infrastructure like roads, clinics, and power lines. Without a reliable foundation to build upon, even the most promising tech interventions are just Band-Aid fixes for deeper problems. I'd love to see more focus on integrating AI into holistic development strategies, rather than treating it as a silver bullet to paper over existing shortcomings.
